The Pediatric Group is accepting all major
Health Insurance Plans!!

As ever, we remain committed to serving the needs of our patients,
allowing access to the doctors, and providing the best pediatric care available.

To help everyone with the transition, please do the following:

• Bring insurance info and card for all family members to your next visit
• Bring a check for your co-pay (we will be accepting payment by credit card as of December 1, 2010)
• Know the name and date of birth of the primary card holder
• Call your insurance company and name us as your PCP (if necessary)
• Inform us of specialist referral needs

We are accepting the following plans:

• Cigna
• CHN
• Beech Street
• Amerihealth
• Aetna
• Healthnet
• Qualcare
• United/Oxford (starts November 22, 2010)

If your plan is not listed, please call and we will help you if possible.
